Foundation Fundraiser

I'm having a fundraiser to benefit the Bobby Sherman Volunteer EMT Foundation. ALL funds (except the shipping cost) will be given to the Foundation as a Christmas present for Bobby and Ward.
Please make out two checks:
- One check for $5.00 per clock for the shipping should be made out to: Chris Lawenstein.
- One check for $15.00 per clock for the Foundation donation should be made out to: The Bobby Sherman Volunteer EMT Foundation.
Please send both checks and rim color choice to:

Please also include the color of the rim of the clock. Your choices are Black, White, Navy Blue, Burgundy and Forest Green. If you want to receive your items before Christmas, I must receive your order by December 20. But I will be taking orders through at least January. If you have any questions, please e-mail me.
The clock is a round 9" diameter clock, battery operated.
